your first appointment
New OB patients should allow one hour for their initial obstetrical appointment and new GYN patients should allow for half an hour for their initial appointment.
New patients should visit our forms page and complete all necessary forms prior to their first appointment. Also, please bring the following documents:
- Please bring a photo ID and insurance card to every appointment.
- Certain patients will also need to provide their referral form and a copy of their X-rays.
If you have any questions for the doctor, please write them down prior to your visit. Also, make a list of all medications and the dosage.
Please provide us with at least 24 hours advance notice for any appointment changes, so that we can better accommodate other patients and our doctors schedules.
While our goal is to see patients at their scheduled appointment time, our doctors are surgeons and can occasionally be called into emergency surgery or to see a patient with a medical emergency. If an emergency arises, we will do our best to notify you prior to your appointment; however, that is not always possible. Your patience and understanding are greatly appreciated.
